New Patient Registration
We accept new patients who live within our practice area (please click on the link below under practice area and input your postcode to find out whether you can register). As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a new patient form and use our patient POD in surgery to obtain blood pressure reading, height, weight etc.
Please obtain your NHS number from your previous surgery to assist us in finding your records (your NHS number may also be on any hospital correspondence you may have).
Please register any new babies as soon as possible after birth (all babies are given an NHS number at birth so please provide us with this information also).
Medical treatment is available from the date of registration; please contact reception for further information.
